We welcome new patients of all ages for general, cosmetic, preventative, restorative and children's dentistry, with treatments ranging from general dental check-ups for all the family to complex full-mouth restorations with dental implants. We send all of our patients a welcome pack when they book their first appointment, which includes a medical history form. You can also print and bring the medical history form with you to your initial consultation. We do have copies of the form available from reception too, so if you haven't had time to complete it at home please arrive a few minutes early and complete it here. The form helps us get to know a bit more about you, what your needs and wants are, and makes sure we are aware of any medical conditions or medications that may affect your treatment. We do our best to run on time, so please arrive in good time for your appointment. You can relax for a short time in our waiting area, then you'll be introduced to your dentist.
